MEDICAL RECORDS

All patients’ medical records will be placed in a secure storage at the end of this year. Your medical records will be stored for 10 years since your last visit. You can contact The College of Physician & Surgeons of Manitoba at

(204) 774 - 4344 and ask for the contact information of the storage place if you require your medical file after December 15, 2019.

 

Or you can send a request by e-mail to dr.longstaffealbert@gmail.com. Please do not leave any sensitive/medical information when sending an e-mail. Someone will get back to you with your request. 

Please note that there is a standard $50.00 - $75.00 fee for this request.
 

Your family physician’s office can obtain the following information from the appropriate sources generally free of charge: consultation reports, X-ray or MRI reports, operative reports.
